While some companies are always looking for innovative solutions to provide value to their clients, others just follow the conventional rules.
What factors are responsible for this difference?

In your opinion, what all could be the reasons of making a poor product or service? Don't you think, all people would like to do a good job and produce good quality? But still you have a lots and lots of poor quality products.
Brainstorm a little bit, and add to the list of reasons, why people produce poor quality.

There are a large number of quality definitions, including the one defined by ISO 9000. But in your own opinion and words, what is quality and how do you define it?
